COVID-19 Vaccination in Toronto

Wondering whether you might be eligible for a COVID-19 Vaccine?

The City of Toronto has compiled a website of vaccine sites and their criteria:  https://vaccineto.ca/sites 

At this moment, some Toronto Vaccine Clinics have expanded their eligibility criteria including CAMH and UHN.

In addition to the provincial criteria, CAMH is also vaccinating patients with mental health conditions, substance use disorders, and dementia. A doctor's note is NOT required, but you could consider bringing in your medication or a pharmacy receipt with your current medication listed.

UHN is pre-registering anybody with an at-risk condition (more details below) if you live in the UHN catchment area or if your primary care provider is within their catchment area. If Dr. Hrabowych is your primary care provider, she is within their catchment therefore you qualify for pre-registration.
