VERIFICATION OF ILLNESS
Guidelines for Issuing Verification of Illness Certificate
A Verification of Illness should only be required in situations where a serious illness or injury results in a significant disruption of academic work.
Students are welcome to consult medical staff at the Student Health & Wellness Centre for information and treatment. We are willing to assist all students to stay healthy throughout the academic year, including at exam time; however, only students with a serious illness or injury will be granted a Verification of Illness Certificate. This policy ensures fairness.
Criteria for OCAD U Verification of Illness Certificate
Circumstances in which verification of illness will be issued:
- Student is seriously ill or injured resulting in significant disruption of academic work.
Procedure:
- Student must be seen at the Health & Wellness Centre while ill or injured.
Once the student has seen the Nurse Practitioner or Physician and the Verification of Illness has been approved, it will be the student’s responsibility to take the certificate to his/her teaching faculty. A record will be kept in the student’s file in the Health & Wellness Centre.
Circumstances in which verification of illness will not be issued:
- Minor illnesses (e.g., common cold, flu, bladder infection, musculoskeletal injury, migraine headaches, etc.), which may disrupt academics should be dealt with between teaching faculty and student.
- You should not visit the Student Health & Wellness Centre for the sole purpose of obtaining a Verification of Illness Certificate.
- Classes missed due to the H1N1 Influenza.
- Missed classes.
- Death or serious illness in the family; documentation from a Funeral Home or hospital is sufficient for consideration by your teaching faculty.
- Past illnesses. Without some form of documentation at the time of illness, a Verification of Illness Certificate will not be issued.
- Obvious injury (e.g., a broken bone in a cast).
- To document a stressful time in the past.
