MAKING AN APPOINTMENT

The OCAD U Health & Wellness Centre is open to OCAD U students during the academic year with modified hours during the summer sessions. For information or to book appointments, please call 416-977-6000, Ext. 260.

Accessing Services

In order to provide a high quality of health care, it is recommended that you have scheduled appointments with the nurse practitioners. To access counselling services, you will need an initial visit with the nurse practitioner. In either case, please call the OCAD U Health & Wellness Centre at 416-977-6000,  Ext. 260, to book an appointment.

Scheduling an Appointment

When scheduling an appointment, students are asked the nature of the visit, so that an appropriate amount of time can be booked. (For example, physicals and initial birth control visits require longer appointment times.) Only general information is required to slot the appropriate amount of time for your appointment. You do not need to disclose the entire nature of your visit when scheduling your appointment.

Plan to arrive at Health Service at least 10 to 15 minutes before your initial appointment to allow for the completion of paperwork. Do not forget your health card. If you are late for your appointment, it may need to be rescheduled. Please call ahead if you need to cancel/reschedule.

If you are experiencing an emotional crisis situation, contact the Health & Wellness Centre during business hours and/or contact any of the following Distress Emergency Hotlines:

Verification of Illness Certificates

A Verification of Illness certificate should only be required where a serious illness or injury results in a significant disruption of academic work. Refer to the Verification of Illness page for full details. A doctor's notes is not required to return to class after time off due to minor ailments or the H1N1 Influenza. The centre does not issue certificates to students who have not been seen or treated at our facility.

Confidentiality

All services provided in the Health & Wellness Centre (health care, emotional support, counselling, education and diagnostic tests) are confidential. Health records are entirely separate from other university documents. No one from an academic or administrative department has access to your health record without your written consent. Also, parents, guardians and other family members cannot have access to any information about you without your written consent, including whether or not you have ever visited the Health & Wellness Centre.
