Health Plan
Eligibility and Enrolment
All full-time students (registered in 4.0 credits or more during the fall/winter session) are enrolled in the Health Plan. Participation is compulsory for full-time students. Part-time students may join in September or January (whichever month studies begin). Family coverage is also available.
Fees and Payment
Full-time students (registered in 4.0 credits or more) will be automatically charged for the Health Plan with their tuition fees. Part-time students can join the plan in the month their studies begin. Full-time and part-time students may add family coverage (spouse or dependents).

Dental Plan
Eligibility and Enrolment
All full-time students (registered in 4.0 credits or more during the fall/winter session) are enrolled in the Dental Plan. Participation is compulsory. Part-time students may join in September or January (whichever month studies begin). Family coverage is also available. Part-time students or students wishing to add family coverage should visit the Student Centre, 51 McCaul Street before the end of the month (September 30 or January 31) in which their studies begin. Coverage is from September 1 (or January 1) until August 31.

Choosing a Dentist
Students can visit ANY dentist of their choice. When visiting your dentist you will need to present their Pay-Direct Card. Standard claim forms are also available at any dental office or can be found below and must be mailed to the address indicated on the form.
While students can visit ANY dentist of their choice, the following dentists have offered to waive the 20% co-insurance on those services that are covered at 80% (i.e., minor restorative and scaling):

Pay-Direct Cards
Pay-Direct cards can be obtained by visiting the SBTA website and printing off a card. Students should present the card at their pharmacy when picking up prescriptions or when visiting their dental office. Please note, some providers will require payment up front and students will have to submit claim forms and receipts for reimbursement from the insurance company. Please be advised that there is a blackout period for pay direct access during the first part of September when the eligibility list is determined and uploaded. Students who pay for services to the service provider directly can submit a claim form for reimbursement.

Students who have paid for services that are covered under the plan can submit a claim with receipts to the insurance company. Claim Forms can be downloaded from the links below and submitted for reimbursement:

Opting out of the Health and/or Dental Plan
Full-time students may opt out of the Health and/or Dental plan provided that they demonstrate coverage under another plan (see below) and complete an online Waiver Request. Students must submit a Waiver Request each academic year. The deadline is September 30, 2011. This deadline is strictly observed.
Waiver Process
All fees, including the health and dental plan fees, must be paid by August 26, 2011, to the Finance Office as per the published fee schedules. Health and Dental Plan waiver requests must be submitted by the September 30 deadline. Upon approval, students who opt out of the health and/or dental plan will receive a credit towards the second installment of fees (if outstanding). Students who have paid fees in full for the year will receive a refund by cheque in November.

Refund Information
Full-time students who waived one or both plans by the September 30, 2011, deadline and who had paid their fees in full for the year, will have refund cheques available for pick up from the Cashier's office located on the lower level of 115 McCaul Street . Refund cheques will be available for pick-up in November. Please remember to bring your OCAD U Student card as identification. Students with outstanding fees will receive a credit on the tuition fee account.
