Special Event Support Request

In order to provide the best service possible we ask that all mandatory fields be filled out and that the request is submitted two weeks in advance. Please make sure to plan enough time for setup and tear down before and after the event. If you are requesting on-site support staff an account number must be provided. Should your event have requirements outside of the scope of this form please describe your requirements in the available field. As a matter of best practice we ask for a single point of contact within your department or group. You will receive email replies to this form, please keep all communication in one email thread.
