Intramural teams are co-ed per the rules provided on the intramural landing page. Each player must complete and submit this form. Winter 2017 intramural games start the week of January 16, 2017*.

WINTER INTRAMURAL: PARTICIPATION FORM

2. Games are generally 30-40 minutes in length. Games can start as early as 3:30 PM, but no later than 7:00 PM.

For scheduling purposes, please select the EARLIEST time you are available to play. Registering with a Team of others? All players must agree on the same start time.

4. I am a "FREE AGENT". I do not have a Team but give permission to be connected (and my provided contact information shared) with other OCAD U students who are also looking to be assigned to a team with similar schedule availability.

I understand that I may be called upon to try and recruit female players if there are not enough players to complete the co-ed team. Per game rules, teams who do not have enough female players will have to default:

WAIVER, RELEASE AND INDEMNITY
Please read carefully:
1. I am participating in “OCAD U Intramural: winter 2017”, organized by Campus Life,
 in partnership with The Michener Institute for Applied Health Sciences (222 St. Patrick Street). I recognize that this program/activity requires some physical exertion which may be strenuous and may cause physical injury, and I am fully aware of the risks and hazards involved.

2. I understand that it is my responsibility to consult with a physician prior to, and regarding my participation in this activity.

In consideration of being permitted to participate in this activity, I agree to assume full responsibility for any risks, injuries or damages, known or unknown, which I might incur as a result of participating.

I hereby release, waive, forever discharge, hold and save harmless and agree to indemnify all of OCAD University and The Michener Institute for Applied Health Sciences and its respective employees, agents, and representatives of and from all claims, demands, damages, costs, expenses, actions and causes of action, in respect to death, injury, loss or damage to my person or property however caused, arising as a result of or in any way connected with my participation to the event, as a participant or otherwise, whether prior to, during or subsequent to the event

I have been offered the opportunity to ask questions about the program/activity and have had all my questions answered in a satisfactory manner by OCAD University and/or The Michener Institute for Applied Health Sciences staff.
