Request for Documentation in Accessible Format
OCAD University is committed to providing accessible information and communication to all members of its community and recognizes that persons with disabilities may use methods other than standard print to access information.
All documents required by the Accessibility Standards for Customer Service are available upon request, subject to the Freedom of Information and Protection of Privacy Act (FIPPA). When providing these documents, the university will do so in the format requested.
If you require publicly available correspondence, reports and/or other university documents in an alternate format, please contact us by email at accessibility@ocadu.ca and let us know what format is preferable.
When emailing your request, please include the following information:
- Your full name
- Mailing address address (optional)
- Telephone number
- Fax number (optional)
- Email address
- Name of required document
- Format Requested (e.g. large print, Braille translate, hard copy, email, html, etc. For video request, please indicate closed captioning or descriptive video)
- Date information is required (plaese note that for closed captioning and/or descriptive video, we require at least two weeks notification per request)
Alternatively, contact Cathy Berry, Accessibility Manager at 416-977-6000 Ext. 2205 and make your request by phone.
In some cases, the university may need to consult with the person making the request to discuss other possible formats in order to provide its services within its capabilities. Alternative formats will be provided within a reasonable time frame, or the university will provide an explanation for the delay if longer than 15 business days.
